3. Understand the basics of the Coleman Model-Care Transitions Interventions

TIME FRAME:

• 5:30 PM to 6:00 PM REGISTRATION/NETWORKING (Cash bar)
• 6:00PM to 6:45 PM DINNER
• 6:45-7:45 PM Program

• $50.00- Registered Nurse Level (Active)
