Private comprehensive care manager/advocate opportunity for a RN experienced with complex care needs of patients with Lupus and Rheumatoid Arthritis
- 8-10 hours/month, includes a minimum of one in-person consultation for updates and assessments per week and other zoom/phone contacts as needed (may include evening or weekend calls/visits)
- Responsibilities: Advocate for patient and family to advise, counsel and coordinate all aspects of care including ongoing communication with patient's providers and care team (HIPPA releases will be provided)
- Caring approach and consistent coordination and support for patient and family to explain and reinforce provider orders, recommendations, new therapies and medications etc.
- Ongoing medication oversight, home environment assessment, timely and direct communication with provider(s) re: changes in patient condition/needs and questions
- Pay: $500 per month.
- Individual RN malpractice insurance coverage recommended
